Male Blepharoplasty: What It Is and Why Being Done Right Matters

Male upper and lower eyelid surgery—also known as blepharoplasty—is becoming more common. Even high-profile individuals are creating buzz around the procedure; for example, some experts believe Bradley Cooper may have undergone blepharoplasty (among other treatments) to refresh his eye area.

Key Considerations for the Male Patient

  • Brow height & shape: A male face typically features a straighter brow (rather than highly arched) and a stronger, more defined upper ridge. Over-lifting the brow or removing too much lid can inadvertently soften masculine features.
  • Hooded eyes: When the skin above the eyelid droops over the lid fold and covers part of the upper eyelid, it’s called a hooded eyelid. Correcting this on a male may require subtle restraint—removing just enough skin to restore a rested look, but not so much that important facial cues are lost.
  • Fat pad removal & skin tightening: In many male patients the goal is to remove excess fat in the lower lid or bulging eye bags, reposition tissue, and tighten skin—but without creating a completely “feminized” eyelid which can be overly smooth.
  • Wrinkles: Crows feet and other fine lines around the eyes can be appropriately aging for male patients.  Subtle use of Botulinum toxin (Botox or Dysport) can help smooth, but not totally eliminate these lines which are often a hallmark of maturity in men.

What the Procedure Involves

During your consultation at SARA Surgery, Dr. Sara Dickie will assess your anatomy, your facial proportions, your brow position, and how your eyelids move.

  • For upper lid surgery: incisions are typically in the natural crease, allowing removal of excess skin and sometimes fat.  Fat can also be repositioned for optimal contour.
  • For lower lid surgery: depending on your needs, an incision may be made inside the lower eyelid (transconjunctival) to remove fat without an external scar—ideal when skin tone is good. Or, an external incision just below the lash line may be used when skin laxity needs addressing.

Recovery involves a period of swelling and bruising (usually about 1–2 weeks), with careful post-operative instructions to support healing.
